I am new to the forum and i have question -

I am currently outside of the US,

In total i was 3 times in the us first time it was sept 2008 for 20 days, then in february 2009 for less than 2 months and from november 2010 to may 2011 for almost 6 months.

Now as i understand i have a time limit of 6 months stay per year with B1/B2 now i have a ticket for tommorow to the US and intend to stay there for a month for my uncle's surgeries (cancer).

The question is whether there is an immigration issue with that since it should be 12 months past my last visit or the 6 months stay limit is per entry (of course, i got out of the us in time and never overstayed).

No, you won't have a problem, especially not as you are a doctor who will perform cancer surgeries on your uncle or are a nurse who is taking care of him after surgery. Just tell the truth if being asked at he airport and bring documentation if possible.

There is no 12 months rule. The IO at the POE will look to see if you are visiting or not. If he/she feels that you have been spending too much time in the US as to practically live here, then you got a problem. Based on your past visits and current wish to be with your sick uncle, you shouldn't have any problems being admitted to the US as long as you can show strong ties to your home country.
