Hi,

I (beneficiary) am preparing our D230, how do I fill-out the names?

On our G235A, the name I wrote was my maiden name, not sure how I should fill-out DS-230, specifically item #1..

#1 asks for Family Name, First Name, Middle Name

#2 asks for Other Names used (if married, maiden name)..

Question, should I write my married name on #1? Problem is, previous forms such as G235A, indicated my maiden name.. Also, my passport also has my maiden name name..

My NBI Clearance I got though shows my married names, because apparently, I was required to enter my husband's last name if married.

If I write my maiden name on #1, should I just write "same as #1" on item #2?

Thanks in advance!

Why don't you update your passport to your married name so your greencard will be in your married name? When we applied for the visa everything was in married name for my wife. It was less complicated that way.

I agree with the above post. When you fill in DS230 on #1 put your married name and on #2 your maiden name. You don't have to explain further why on your G235A you wrote your maiden name 'cause NVC will see that the case is for the same person since your maiden name will be indicated on DS230 #2. Remember the process won't end in USEM, there's still more process even when you already get to US. Better keep things in order while you're still here.

It took my wife 2 weeks from the time that she paid the fee until she got her passport back. That was just standard processing. Pay a little more and it gets expedited. Check out the DFA website. Make an appointment so you can submit the application & pay the fee all in the same day. My wife had 2 make 2 trips to manila for her passport because she didn't know she could get an appointment. Without the appointment the line can be quite long.

It is much cheaper to renew/update your passport in the PI than doing it in the US.

How long would it generally take to update a passport in the Philippines to my married name?

I would suggest that you visit any DFA office and talk to a representative and explain your case, 'cause now they are a lot of delays for processing of passport even if you'll just update your lastname. You know how it works here in PI.
