Question 2 - As far as I can see the UK passports do not say the place of issuance so is it ok to just write n/a?

Also question 36 - Has anyone ever filed an immigrant visa petition on your behalf......Im applying for K3 so does this mean anyone other than my husband? In which case i say no?

i put Newport, Gwent for the place of issuance, that's where my mum said they get sent... I put my fiance's name for the applying for an immigrant visa bit, as the DS156 is a non immigrant form, it's quite a general form for a lot of circumstances.
