Question 18 on DS 156

This may be a dumb question but I want to make sure I get it right on the DS 156. 18. Spouse's Full Name

(Even if divorced or separated. Include maiden name.) She is divorced and thinks she puts her ex husbands name there but I think it is where she would have put her married name if she would have used his last name. I have tried to research this but I find it has been answered both ways also. Does anyone on here know for sure? Thank you Daniel

its the ex husbands name that's how i answered it doesn't ask for your married name it asks for the spouse. maiden is if the applicant is a man and was married to a woman its that woman's name
