Hi there,

I received the same checklist as you saying that if you have not submitted you need to bring it. I handed over my packet at my POE in Toronto which included my medical and when I filed for my AOS application all I sent them was a photocopy of the vaccination supplement sheet that I received after my K1-Medical. I did not go to a civil surgeon here in the states once I got married and I did not fill out the I-693 form. I would just take a photocopy of your vaccination sheet to your interview. I didn't get asked for it at my interview and got approved for my Greencard so I am assuming they had all the medical information in the file from my K1 Visa. Hope this helps Good LUCK :)
