Hi,

I'm putting my N 400 package together and wasn't sure what to enclose to the package.

Could anyone who file for N 400 based on 5 years (divorced) share with me your check list?

So far, I have my tax returns (3 yrs or 5 yrs?). My divorce paper, copy of my green card, pictures.

Do I need to add my bank statement or my utilities bills? I travel oversea for work approx 2 to3 times a year; approx 12 trips for the past 5 years. Trips last 2 to 3 weeks each time. Do I need to provide a letter from my employee?

I also wanted to change my name, resume to my maiden name. Can I simply check name change on N 400 form? Do I need any special form for name change?

Mamata, you only need to submit your application, divorce decree, marriage certificate (if remarried), front and back copies of your green card and drivers license (to avoid getting the yellow letter later in process ).

You can take your tax documents, proof of travel dates and details on documents showing previous marriage entered in good faith (only if your GC was obtained by marriage to a USC) later when you will be called for the interview.

Yes you can. Just a quick point to note..Changing name via N-400 will require a separate judicial oath as opposed to an administrative oath and could take a long time (even 6+ months in some cases depending on your DO). Only benefit doing it via N-400 is that you save the 75 odd dollars that would cost if you do the name change separately, but would take only 2-3 weeks if done that way.
