so ive heard different things from different people and in the guides about the immunization document/ medical for the AOS.

ive read that because i had my medical (in australia) only a few months ago that i wouldnt need another one for my AOS, and all i have to do is include the ds-3025 i recieved with my medical.

is this true? i ask because in the guide it says to go and see a civil surgeon and get them to transcribe the record, at the same time people have told me i dont need to do that and just to include the copy of the ds-3025 i already have.... im confused.

And you should be confused. Some can read the instructions and interpret that no 693 is required and they don't submit any and have no problems. Others get RFEs for the same.

I interpret the instructions differently and I followed the VJ Guides and even though having the 3025 for both, we had both transcribed by a CS onto the 693 forms (ask for that transcription service only and don't cave in to another medical). We had no RFEs.

So, if you feel lucky, don't do it and just be ready to do it if you pull an RFE.....or play it safe and do it now.

My advice, get the 693 also.

The thing is some get by with just the DS 3025 and don't have an issue but it is advised to get all your vaccinations transcribed over on to the I-693 vaccination only part. No you don't need another medical but rather than get and RFE is safer to send the DS-3025 along with the CS I-693 in sealed envelope. Totally up to you.I sent both but hey they lost my medical so I had to send off to Sydney for another one lol. Cost us $50 to get the DS 3025 transcribed over to the 693.
