Hello guys!

I received an email from NVC tonight. It is asking me this:

We have reviewed the documents which you submitted in support of an application for an immigrant

visa. Please review the information listed under each applicant's name and provide any documents

which are indicated as missing or need additional information. You must submit the required

information. Failure to provide the requested information will delay your immigrant visa interview.

______GUZ P3 SUPPLEMENT:___________________________________________

For each case, please submit a Supplemental Information sheet that lists all traveling beneficiaries. The

sheet can be found and downloaded from the following link:

http://travel.state.gov/visa/immigrants/info/info_3830.html. The Packet 3 Supplement sheet should be

signed by the Principal Applicant (or parent if the child is under the age of 14).

then I logged site as it mentioned about to download form, and it came out like this :

In order to reduce delays in processing your immigrant visa case, please complete this form with the following information and return it, along with other completed Packet 3 documents, to the National Visa Center:

1) GUZ / GZO case number

2) Visa Category

3) Name of each applicant in English and Chinese characters

4) Current mailing address in Chinese characters with postal code

5) Date of Marriage (if applicable)

6) Local Telephone Number

Signature: ______________________ Date: _________________________

I sent out a completed DS-230 form, and they received on 3-7-2100.so my question is which form I should use to completed what NVC requested from this email? Because the site i went it there has no form that i could complete for my hubby, it only asks for signature and signed date , no where that i can fill out information for my husband. or my husband has to bring this form to his city local agency to fill it out for him, and mail it back to USA? I am confused.

Fill the Packet 3 Supplement sheet which you downloaded from the http://travel.state.gov/visa/immigrants/info/info_3830.html. Also see the attachement. You can rewrite the form by Word and fill it out like this:

1) GUZ / GZO case number: your case number

2) Visa Category: your visa type

3) Name of each applicant in English and Chinese characters: Your husband's name

4) Current mailing address in Chinese characters with postal code: Your husband mailing address in Chinese

5) Date of Marriage (if applicable): the date when you guys are married

6) Local Telephone Number: your husband's phone number

Then fill it again in the Chinese section in Chinese. Then have your husband sign it, scan it and then email to NVC(NVCElectronic@state.gov) if you are doing electronic processing. Otherwise, you mail to NVC.

Is this for K-1 visa only because I am doing a CR-1? The form Packet 3 Supplement sheet.

04/06/11 Automatic recorder on the NVC machine stated that my package was received this date and have to wait 6-8 weeks for next responds

04/16/11 RFE issing my spouse passport biopage

04/18/11 Email back the missing paperwork for NVC

04/19/11 Email from NVC they recieved the RFE I sent.

04/20/11 Spoke with NVC they say have to wait 20days for change

05/16/11 Spoke with NVC they say "case is complete, and that their interview appointment is full for June- to call back in June 17 for a interview appointment time for July most likely."

05/16/11 SIF on the payment portal website

05/20/11 Interview letter arrived via email for June 15 interview date in Guz

06/13/11 Medical Test today

06/15/11 Doc intake

06/16/11 Interview day waited for my wife at the embassy

06/16/11 APPROVE

06/18/11 Visa in her hand at China post office

6/23/11 Land in US at San Francisco airport

7/11/11 Waiting for her SSN card, received her Green Card Already
