Medical Exam / Vaccination Form

As some of you know, my husband did not receive a copy of his vaccination form, and needed a few vaccinations after his K1 med. exam anyway, so we've been shopping around South Jersey for a Civil Surgeon to fill out the form.

For the record, none of the offices we called (two in Cherry Hill, one in Camden, one in Lumberton, and one in Mount Holly) would do the vaccination form without re-examining my husband. The one in Mount Holly wouldn't even see him without us guarenteeing we'd paid the full amount of the exam.

So, we made an appointment in Lumberton (because they have evening hours, so I wouldn't have to take off work to drive my husband to the exam) and in the end, the Civil Surgeon was sweet and seemed rather knowledgable, and even with the full exam, we got off pretty light:

$60 for the exam

$80 for the innoculation David was missing

$20 for the TB test

As the doctor is taking on David as a regular patient, temporarily, he's running a full physical as well and taking the results from that blood test for the medical exam -- which means my insurance will cover it. Over all, even though we paid a good deal more than we'd expected when we began this process, I feel we got off pretty lightly, especially when I've read about people paying $125 to get a form filled out.

I paid $400 just for shots and the supplement. So, yes, you got off easy.
