Applying for Redress via DHS TRIP

Hello!

Has anyone applied for redress using DHS Trip? I'm looking for some sample wording for the DHS TRIP form. I was denied entry into the US nearly 4 years ago and this Summer when I was coming home to the US after being in Europe I was held back by Customs for an hour and almost missed my connecting flight. When I questioned why I was being held back they said it was because I was denied entry years ago. This was the first time in 4 years I was hassled because of this. The officer gave me information on applying for redress via DHS TRIP. I'm just looking for some tips/sample wording on filling out the form. It looks simple enough, but I want to make sure I do it right.
