AOS app packet...how many copies?!

My husband and I are getting ready to send in the packet for his AOS (hopefully tomorrow!) and someone just mentioned to us that we need to send in 4 copies of each document!? Can that be true?

We can't find anything about that in the instructions, but just wanted to be sure!

Also, the fee of $1070 includes the cost of the biometrics or is there and additional fee for that?

I'm sorry if these are ridiculous questions!

We just want to be sure we do it right!
