Where do I send the forms to?

I am adjusting my status (I came here visiting the US from Canada and am getting married in 2 days!), and all the forms are filled. However, where exactly am I sending the forms? Like what address? Also, do I need to get the I693 (Medical Exam) done before I send in everything, or once I get to a certain stage, they'll inform me to go get it done? Thanks everyone, I just want to make sure I'm doing everything right!

Yes you have to have a medical exam. You also have to include your marriage certificate.

edit to add - they dont tell you where to have it done you have to go get it done at a civil surgeon in your area. Use the USCIS civil surgeon locator to see the approved ones in your area.
