question regarding affidavit

hi to all, i have question regarding social security number on affidavit.is social security number of the person writing the affidavit has to be there? i was reading the instruction on i 751 and it said that , the person writing the affidavit has to provide full name and address, date and place of birth, relationship to you or your spouse, and full information and complete details explaining how the person acquired his or her knowledge. but it does not say that the person writing the affidavit also have to provide his or her social security number. i was reading in some forum that affidavit should also contain their social security number. i am cofused .please help. thank you.

No, name, address and phone number should be just fine. They only want a good way to contact your signer (if they ever do so).
