Non Impediment HELP!!!!

My fiance went to go get his non-impediment. He now says he has this form to fill out...can someone tell me if its right cuz I thought it was a simple piece of paper to get(needing only Birth Certificate and Passport), Does he have the wrong thing?

I especially don't get the last one, how can I get an affidavit, I'm not there....

He says that to form wants:

1)Name

2)DOB

3)Usual place of residence in Jamaica

4)Name, Address and nationality of the person you intend to marry

5)Name and address of 2 Jamaican residents willing to provide references on your behalf

6)Both person should complete an affidavit available at the ministry, in the presence of a justice of the peace

It almost sounds like he was asking about registering a marriage, not a no impediment - I have never heard of that before - You might want to call the RGD - he needs an affidavit from them that they searched their records and did not find a marriage recorded - the name of the person he is marrying is irrelevant.
