Short version: wife doesn't have original passport, as she surrendered it when it expired and a new one was issued. On the I-485, I wrote "see I-485 supplement" and then am including this as the supplement. Any comments/critique? I don't know how formal to be with the language, or how much detail to give...

Part 3, Section A:

Non-Immigrant Visa Number:

The beneficiary’s non-immigrant visa number is unknown, due to the fact that when the beneficiary’s original passport (containing the necessary entry stamp and visa) expired in 2008, she applied for a new passport at the Czech Embassy in Washington, DC. When receiving the newly-issued passport, she was asked to surrender the previous one, but was not advised that she might require copies of the information contained therein for later adjustment of status or other official purposes.

The visa was issued at the US Consulate in Prague.

The beneficiary’s J-1 visa allowed her participation in the au pair program operated by Au Pair Registry (formerly known as Au Pair Program USA, now known as Go Au Pair), as indicated on the beneficiary’s form IAP-66 (attached). She entered the USA twice on the multiple-entry visa, on 11/27/1999 and on 7/20/2000.

Date Visa Issued:

We do not know the date of issue of her non-immigrant visa, for the same reasons that we do not know the non-immigrant visa number.

I think that's the biggest single remaining issue, so when that's done, we should be good to go!
