I have a DS-3025 that shows 'vaccine history complete', but one item is 'insufficient time interval'

I have all of the pieces for the AOS packet except for my I-693 completed by a civil surgeon. I know most people will say I should send an I-693 regardless of the DS-3025, but I had a few questions:

1. The DS-3025 is checked 'Vaccine history complete for each vaccine, all requirements met', but it also has a section after MMR where 'insufficient time interval' was checked and then stamped over. I am not sure if that stamp is intended to negate the check or verify it. I have uploaded an image of the DS-3025. Any thoughts?

2. If we submit the AOS, EAD, etc without an I-693 and then get an RFE for the I-693, will this hold up the EAD? My wife really needs to get back to work and we are anxious to get the paperwork moving for that. If the medical RFE doesn't mess with the EAD, then we might just try it.

3. We also got a letter from the Tokyo Embassy when she got her visa with a short paper showing "Finance Visa - Immunization Notice This notice is to advise that you have not fully met the requirements of Section 212(a)(1)(A)(ii) of the Immigration and Nationality Act and that certain vaccinations may need to be obtained prior to your adjustment of status in the United States. Foreign nationals who are issued United States fiance visas are required to fulfill the vaccination requirements of 212(a)(1)(A)(ii) either at the time of application for non-immigrant fiance visa or at the time of adjustment of states in the United States (unless the USCIS otherwise waives the requirement". It was photocopied on the back of another sheet about conditional residence based on marriage and did not seem like an official notice. Is this somewhat generic or does it indicate an I-693 and new exam is needed?

My wife called Kobe Kaisei Hospital and found out the stamp over the 'insufficient time interval' stamp was intended to show that it was mistakenly marked. I have requested a new DS-3025 to hopefully avoid any confusion that this mistake might cause with the AOS or I-693.

Does anyone know if the an RFE for the medical would delay the EAD approval?
