I am a Permanent Resident here in Florida. My hometown is Philippines. I am planning of going home this year for holidays. I wanted to renew my passport but in my passport I put on my last name. I got in the U.S as a K-1 Visa(Fiancee Visa). I got married here and now I wanted to change it into my husband's last name. In the requirements to renew passport it says that I need my "Affidavit of Immigrant Status" and I have no idea where to get the form. I am lost right now.

If anyone know how I can get the form....pease help!!!

Affidavit of Immigrant Status. You have a PR card, right?
