Hi all need some advice on the DS-230 part 1 form.

Hi.... I have approached no.25 on the DS-230 part 1 form. Where it asks for me to lists dates for any visits i hav taken to the USA. I am wondering what to put where it asks type of Visa used. I am from the UK so i am on the visa Waiver list. All i have filled in was the card they give u on the plane just before landing. Do i put that card down, for where it asks what type of visa??? i cannot even remember what the name of that card is. I would be very grateful if someone cud passs on some advice for it.. Ty

I did the K3 visa but I remember writing that I entered on the VWP

VWP - Visa Waiver Program - sounds appropriate?

The card is I-94, Arrival/Departure Record, and it's not a visa or a visa-substitute.

Thanks you Guys ... that is very helpful to me not long got home from work & decided to crack on with these forms. Im sure i will be asking heaps more questions in time ..lol
