First, I think she needs to renew her passport. Check out this link, it's where you can find everything that both you and your fiancee has to prepare for her interview.

She needs to bring all of the documents on the day of her interview. By the way, she needs to pay the visa fee ($131) but converted to peso, in Banco De Oro bank (not sure if all branches accept payments), 2-3 days before her interview. After paying, they will give her one form of DS 157. She can download another copy.

After your fiancee's medical, St. Lukes will take her passport. They will be the one to forward it to the USEM together with her medical results.

I suggest for her to get a new passport before having her medical.

I don't think you need a new passport. The requirement for a passport is:

a. PASSPORT. You must have a passport valid for at least six (6) months. If you do not have one, you should apply in person at the Passport Division of the Department of Foreign Affairs (DFA), Roxas Boulevard, Pasay City, Metro Manila. For passport renewal, you can call the DFA Information Center at 02-737-1000. The DFA web site is http://www.dfa.gov.ph.

Passports from the PI are good for 5 years. I would think that they only want to make sure that the applicant will have a valid passport when the 6 months for leaving to the USA has expired.

I hope this helped. Good luck,

Don
