Does the Bogota Embassy require ds-156 form filed or do they use something else ?

actually, the system to apply for a tourist visa has changed. There is a new application form, effective March 1st, 2010, individuals applying for a non-immigrant visa at the U.S. Embassy in Bogota will be required to submit a new electronic application form, known as the DS-160. More info here
