We're preparing for our AOS packet to send off before my Husband's medical expires in June or July. He is coming from Egypt and he tells me that the Doctor working with the Embassy doesn't keep medical records. It is like getting a cup of coffee to go business so the only records were the records put it the sealed envelope to be given to the Embassy and a copy of the vaccinations.

Our question is what do we need to show to the civil surgeon? Just the vaccinations or the full medical that we don't have an extra copy of? Also can the civil surgeon accept a release of information to obtain a copy of the full medical from somewhere? Does anyone know where the full medical that was in the sealed envelope goes?

Note* We have the DS-3025 & it's dated July 8th, 2009.

Civil surgeon only needs the DS 3025.

Check the list of CSs in the AOS section for civil surgeons that won't rip you off.

I hope you can find a CS in your area that won't try and do the full medical.

LOTS of CSs are rip offs or not well informed.

Only thing they have to do is transfer the vaccination info from the DS 3025 to the I-693 and give it to you in a sealed envelope. Have them give you a copy of the I-693.

what if the vaccinations on the ds-3025 were incompleted? can you take the required vaccinations right there at the CS's office and then have him transfer and sign everything onto the i-693 form?

Probably cheaper to get them at your local health department or your own doctor. That's what we did. Just take a copy of what you get to the CS.
