If your K-1 medical exam is more than 1 year old at the time you file to adjust status (AOS), it is no longer valid. You will need to do a full medical examination at a local civil surgeon (CS). Go to http://www.uscis.gov, and search to find a list of civil surgeons near you. Phone around to find the best rate for the examination. Expect to pay at least $150-$200. You will need to bring records of any vaccinations you have had, so that the CS can confirm that you are up to date.

After the examination, the CS will record your results on a Form I-693, and seal that form in an envelope. You will need to include this sealed envelope with your AOS application. Make sure you do not open this envelope - just include it in with the rest of the papers you send for your I-485, I-765, and I-131.
