My USC husband is filling up his G32A...Just to make sure what we are doing is right, what should he write down on

Curret husband or wife(If none so state)

Family name(For wife,given name)

If my married name now is Connie Smith and my single name was Connie Santos, should he write Santos Connie since it is written Family name (For wife, given name)

Pls help thanx

Family name(For wife,given name) - Connie Smith (you will put your single name when they ask "other names used", now your family name is Connie Smith, your single name is called maiden name)

If my married name now is Connie Smith and my single name was Connie Santos, should he write Santos Connie since it is written Family name (For wife, given name)
