arggh cover letter and quick question

Where can i find a good expample of the cover letter? i have found 2 on here but they have confused me..i really need to get this done tonight so i can get it printed off but im just to stupid

also i have read everywhere that we only need ot put one copy of each the G-325A in??

Someone please help i really wanna get this done
