Civil Surgeon in Michigan?

Anyone know of a civil surgeon in Michigan that would do transciption only? We called several of them already but they kept on pushing the full medical exam charging us from $200 to $500!!! They said they wont transcribe unless I do the full medical with them. We kept on telling them I just had my full medical and vaccinations (MMR and Tdap) last October so I dont need another full medical. One also said I need to have a TB skin test also, is this true?

Please if anyone know of a civil surgeon in Michigan that would do transcription only, kindly post it here.

I used the civil surgeon locator for my area and then called around for pricing. We found a doctor about 1 mile from our house that charged us $70 for doing the paperwork and giving Claudeth the tetanus shot that was available at St. Lukes in Manila.

We went to Dr Haddad in Dearborn with just copies of vaccinations from our heath department. No physical was required because she was a K1. He charged us $75, cash only

Thanks for the replies. Too bad I wasn't able to call Dr Haddad first. We just went to see Dr. Putrus in 14 E. Mile, Warren this morning. He did a blood test and TB skin test on me so I need to be back in his clinic on Saturday morning for the result and for the certification on my I-693. He charged $135.
