Hi,

We are applying for AOS and will be having my wife's follow up vaccine. We called one of the civil surgeons here in Oklahoma today and we got an appointment for everything we need to be done. It's so sad that they are charging us an "amazing" price like $500 all in all for all the vaccines. Now, I realized if we can just go to a local health department here and have it them since that's like half of the ticket that we purchased last year to go to the Philippines. I guess we could just print out the I-693, filled it and then have the civil surgeon transcribe it. Do you think that would work?

find a local health center for the vaccines and just have the i-693 transcribed/signed by a US Civil surgeon (u can print the i-693 form but dont sign it,it has to be done infront of the CS)... they dont necessarily do the vaccines exclusively...

my vaccines was done by the same US CS because they happened to be the cheapest of all 17 other local health centers i called.... plus the i-693 form at $15.00

wow! that is too much. i went to walgreens and had a tdap shot for 65usd. then i went to a civil surgeon to have the vaccine record certified and i was told that i still lack another shot -- mmr (now i wonder what shots were administered to me in st. luke's. i had 2 over there.) anyway, as i was already there at the civil surgeon's clinic, i had the mmr shot (95usd) and paid for 70usd for transcription. i suggest look for another civil surgeon in your area.

Thanks for your reply.. I had the HPV and Varicella in the Philippines but they said I need to have the follow up for that after two months here in the US. Anyway, it's cleared enough that I don't have to do my vaccines on those assigned civil surgeon cause I think they are taking this advantages and charging us too much. I guess it's everyone's problem specially during AOS. Oh well, local health department is my choice. Thanks again and God Bless ! !!

My CS actually suggested that I go to the Health Department for my vaccination (Tdap) if I would like to save some money. So, to answer your questions, go to Health Department and get the shots or go to your family physician if you have insurance to cover them. Then take the record to your CS and have him/her to transcribe it to I-693.
