When I sent my case, I only included DS-3025 as proof of vaccinations instead of the I-693's vaccination supplement.

Today I recieved an RFE that needs a response by December 30th! It states that I need a "NEW MEDICAL FORM I-693"

It also says "A review of the record indicates that you entered the United States pursuant to a valid K-1 nonimmigrant visa. All USCIS records relating to you have been reviewed. The U.S. Department of State medical forms DS-2053, DS-3024, DS-3025 and DS-3026 are not in any of those records.

I have a few questions:

1. is it likely that I will be able to get a medical in 3 weeks surrounded by christmas?

2. do I actually need a full medical or just the vaccination supplement?

3. is this typiccal of a case where they have lost my medical records and contacting a congressman could recover this (as in another thread), and is this way feasible within the 3 week timeframe?

4. Could I request the doctors office in london fax/mail a copy of my DS-3025 if that is the document they misplaced?

Not a good birthday/christmas present .. but thanks in advance for any advice you guys have :(

I can answer the first two only based on experience:

1. You can get everything done within a week if you make an appointment as soon as possible. I remembered I called on Monday and got my appointment for Wednesday. If my TB Skin Test didn't turn out positive, I would have received all my I-693 paperwork back by Friday but I had to do my X-ray at the Health Department (to save the $80) and waited until the next Tuesday to get my X-ray result and I went back to the Surgeon's office to give them the results and they signed off everything on the spot! So, with all the trouble, it took me 8 days to get everything done! Also, to take note, if you are asked to do the TB Skin Test and if you are tested positive, it is required by law here in NC that you have to report that to the Health Department.

2. If your K-1 medical is less than a year, you don't need to redo everything. Just complete whatever vaccinations that you're missing and transcribe whatever on DS-3025 to I-693. That's it. The nurses at the civil surgeon's office that I went to seemed to know what they're dealing with. They kept explaining to me that I didn't need a full medical if my medical was less than a year old and I kept telling them that mine was more than a year old! So, yeah, they are honest!

The forms listed are the DOS forms used by panel physicians abroad when a medical is performed.

The obsolete Optional Form-157 (OF-157) has been replaced with the new DS-2053 and associated worksheets (DS-3024, DS-3025, and DS-3026)."

Looks to me like they have lost your entire medical exam. If others have had good luck getting assistance from a congressperson, then you could try that avenue. You don't have much time to respond though should that fail.

They lost your medical you gave them in the brown envelope at POE.

The clinic in London does not keep files of your records.

Yes, you need a full medical, not just the vaccination supplement. There is no record of a medical for them the approve you on. You can either find a way to round up that brown envelope or pay for another medical and get on with it. It's an unfortunate situation and shouldn't have happened.
