Checklist Question Packet 3

Just wondering about the Checklist that comes with Packet 3:

1) Does it need to be returned with the signed section?

2) (if so)What do you do about "medical" when obviously you wouldn't have done that until closer to the interview, check it or leave it blank, put in N/A or...?
