You can find one in Spanish in the list of documents of the US Embassy in Bogotá web site. It has a 09/2009 date but you can still use it since that's the version the embassy is offering. Or you can use one in English if you prefer.

Does it matter if I use the english one to turn in the embassay or must I use a spanish one since it is a spanish country?

It doesn't matter which one you use but if the beneficiary uses the English form and does not speak English, he/she needs to write down the name of the person who helped him/her fill it out on question #44.
