OK, I'm ready to print this form out but I'm stuck on question 30 "Have you ever been issued a visa". My fiance came here to visit me for the month of August from Italy and presumably had a "behind the scenes B2 because he came as a tourist, but was never "issued" a visa b/c the US and Italy have a tourism reciprocity. What is the answer to that question? And if it's "yes" B2, what do I put for the date issued? The date he entered the US?

Unless your fiance applied for and was issued a US visa by a US consulate—which fact should be easy enough to verify, as it would be affixed to a visa page in his current or previous, expired passport—then the answer to this question is "no."

Visitors from Italy (and other countries that participate in the VWP) can visit the US without a visa under the terms of the Visa Waiver Program. Entry under the VWP is in lieu of a visa, not a "behind the scenes B2" or any other kind of visa. Citizens of those countries who do not qualify under the terms and conditions of the Visa Waiver Program must apply and be issued a visa by a US consulate.

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

OK thanks. I will just put "no". By the way, you told me how to get my tax return transcripts by phone from the IRS and you were right, they came really fast. So thanks so much for that too!
