That's the only form.

I did read. I wasn't comfirm(can't believed my eyes earlier that I only need I-751) until I read your post. Thanks.

Life is not a granting factory, according to my colleague.

Note that it is NOT simply a case of submitting the I-751 form on its own.

Download this document which gives you instructions not only on how to complete the form but also lists the additional documentation that MUST accompany it.

Good luck

PS: I realize a link above takes you to the VJ guide but, with all due respect to VJ, I would always download the USCIS guide in the event something has changed.

Note that it is NOT simply a case of submitting the I-751 form on its own.

Speaking strictly of forms - what others are you finding in the instruction pages of the I-751?

I would like to believe the OP is not under the impression that he/she is only going to need to submit the completed I-751 form and nothing else (i.e., supporting evidence) nor was that suggested.

FYI - Most of the forms on VJ's Download section are directly linked to the USCIS site - its not a bad idea to download them directly from USCIS though and I doubt anyone on VJ would be offended by this suggestion.

:time: <---this not only helps you, it helps others as well..
