Hello! Are there any nurses here? RNs? USRNs?

I finished college in Philippines. I'm now living in San Diego with my hubby.

I'm planning to take NCLEX that's why 2 months ago, I requested my school to send my requirements to BON.

and now I'm confused.. what is the next step? I'm not yet done with my fingerprinting for NCLEX.

I haven't sent my application form with pay check.. coz I really don't know what name should I use..

In my diploma and transcript of records, I'm using my maiden name.

but now, in my id and employment authorization card, it's in my married name..

What should I do next? I believe my school papers are already in BON office.

and another question: where should I address the paycheck and how much is the application and examination fee?

any advice will help. salamat!

If you already have the NCLEX application form and had requested your school for the requirements to be submitted to CA-BON , then you already have it there written on the 1st page of the application form the amount that you need to pay for the NCLEX.

Please read carefully the application form.

Application fee $75 + Fingerprint card $51 = $ 126

For the Fingerprint= since you are already here in the US, then you can use the "Live scan" instead of the "card".

There's also a form there where you can request a change of name for your application form-- if you would like to use your married name, they will only need proof/documents like Marriage Certificate and you may also send a copy of your GC or your driver's license (if you already have one).

In my case, I applied for NCLEX when I was still single (TOR/Diploma in my maiden name) but when it was time to take the exam (I am already here in the US), I requested for a change in status and name (you can download the forms), I mailed the form with my marriage certificate and other supporting documents and you can also call them @ (916) 322-3350.

Hope this info helps. My suggestion, enroll in one of the on-line NCLEX exams review courses like the one NCSBN is offering, they are very similar to the actual NCLEX items. Good Luck on your application and exam.

17 June 2010 - Oath Taking (Memorial Auditorium, Sacramento, CA)
