Guidance and Counseling Form

Timeline

I now understand as a K1 Visa we do not need the Immigration Data Summary. A number of you replied to my last question about that, so now I realize it is only for K3.

One of the people who helped with a reply mentioned the Guidance and Counseling Form, and that is another one I can't find on any web site to download and fill out. I'm guessing that form is provided at the Smef-Cow center for the counseling session; am I correct?.

Am I also correct in my guess that after the Guidance and Counseling session as Smef-Cow, which is located in Quezon City, she must get to the CFO Center in Manila to show the Certificate and get the sticker put in her Passport?

They provide the forms at the seminar, just bring necessary requirements and docs. As far as I know, if you will attend the guidance and counseling session at SMEF-COW, they can also give you the sticker/stamp afterwards as long as you already got the visa. But if you will go to PRISM, which is in Taft, you will have to go to CFO main (which is not so far away) for the sticker/stamp. I'd suggest that if you are closer to SMEF, go there as soon as you received your visa. :thumbs:

hi, the mentioned form is given on the day of the seminar. your fiance can go to SMEF-COW in Quezon City for the seminar and sticker as well. just please tell her to bring her passport (with visa, if she already has it), payment (Php 400 and Php250), pics of two of you together, your landline number in US is a must and your mom's maiden name will be asked as well.
