Hello, I came in to the US on a K1 visa earlier this month and filing for my AOS, one of the requirements was to submit the I-693 Medicals with Vaccinations records.

Before i came to the US my medicals were conducted in Chennai, India by a civil surgeon listed by the American embassy and he said i don't require any vaccinations and he mentioned the same in my DS3025 forms. He gave me a copy of my Medical transcripts which i have bought along with me. Do i need to submit the same along with the I-693 or do i need to visit a civil surgeon listed by the USCIS and ask him to transfer the details on to the I-693?

I have had my vaccines when i was really young but i don't have any recollection to which ones i had or documents pertaining to them. Do i need to get the vaccines again? or as the civil surgeon in Chennai suggested that i don't require the vaccines?

What should I do? Please if any of you'll have had any experiences with the vaccinations, please reply back or suggest a someone i can speak to.

You need to take the DS-3025 to a US Civil Surgeon for transfer to I-693..

I-485 specifies I-693, NOT DS-3025.

US Civil surgeon will determine if you need any additional vaccinations for your age group or not.

My husband was in the exact same situation as you. (Maybe even same doctor - Sanjan John.). He got the copy of medical from doctor, who said no vaccinations necessary. What doc meant was no vaccinations necessary to get K1 visa to come to USA. But for AOS, yes vaccinations are necessary and they have to be noted on form I-163 by a civil surgeon. After coming here to USA, my husband got his vaccinations through a local community health clinic (cheaper than at civil surgeon) then brought the proof to civil surgeon who gave the sealed paperwork (with copy to us). We sent that with AOS and got approved, no RFE.

My husband also had vaccinations when young but had no proof and no recollection of what they were. As per advice of civil surgeon, in USA he had Varicella, Tetanus and MMR (Measles-Mumps-Rubella). Two doses each required of MMR and Varicella, several weeks apart.
