Question about I-693a name/address

My wife and I are working on our AOS and we came across a question.

She had the vaccination record was done right after she got to America before she changed her name with the Social Security office. When we went to the Civil Surgeon the name and address that we wrote on the vaccination document matched the one she did in Japan. The address and name are different from the AOS application (which has her new name and our current home).

The address she put in Japan has my parent's house because we were moving out immediatly after she got here.

My question is does this matter and will we need to change the vaccination document from the Civil Surgeon to match our AOS application (I-485)?
