DS 169

Hello All...help. Packet 3 arrived at the embassy on February 5th, 2009. After a few calls and 7 weeks of waiting we have been told we are missing Form DS. Has anuone else had this problem, or know where we can get this form?

I *think* that's one of the two checklists in P3. Googling brought up an old version: http://www.noueltaxrefund.com/State-Form-OF-169.pdf You might email London and ask them if this is acceptable.

If you go to the link given in your Packet 3 the check list and all the forms required are in the upper right hand box. There is no DS 169, it's not even on the embassy's list of forms.
