several AOS related questions (forms)

sorry I don't have time to search since I am very busy at work every day and we have no internet at home for the time being...

my (now) wife arrived on a K-1 and we got married the day after she got here..now we are working on her AOS

general question on all forms #1: do I put her Family Name as my last name, or her maiden name? there was only one form in the packet that asked for Maiden name...

general question #2: Alien number? I see on her I-94 what looks like an alien number: A########, but it doesn't say that is specifically it

Form I-765: Employment

question: on the Guide for getting a SSN, it mentions this employment form. should I just file this form with the AOS or with the SSN?

Form I-485: Register Permanent Residence

Nonimmigrant Visa Number -- I assume this is her K-1 Visa #.. and on her K-1 Visa there is a Control Number, and some numbers by themselves in Red.. my wife believes her Visa # is the number in Red, is that correct?

and finally, we are all around confused about the medical part of this process. she had a medical examination in Moscow, and she had some sort of document about it..but the Customs lady in Denver took the entire sealed envelope with all the evidence and papers. we have no proof of vaccines, or anything medical related basically.

it seems she will need to get an examination here (vacciness too?) and file the I-693 since we have no documentation.

finally, how do we find the CIS accredited doctors? do we find out after we file the AOS packet, or before, etc...

thanks greatly in advance for any help.. obviously this is all very important for us :)

another quick question...

her birth certificate: it's not exactly easy to come across a certified russian-english translator. she is an english teacher herself and can do the translation flawlessly -- is this acceptable? has anyone filed a birth certificate without a certified translator?

Timeline

If your wife intends to take your name legally, then you should fill everything out from now on with her married name on it. If she is going to keep using her maiden name legally, then she should use her maiden name from now on. It's her choice.

The Alien # starts with an "A", followed by either 8 or 9 numbers.

You file the I-765 (Employment AU\uthorization) with the AOS application, not with the SSN application.

Non-Immigrant Visa # is the red number.

If it has been less than one year from your wife's medical exam in her home country for the K-1 Visa, then another medical exam is not necessary. The Vaccination Supplement is the only thing required (plus any vaccines that are still not completed). If she has a copy of the "Vaccination Worksheet" from her medical exam for the K-1, the Civil Surgeon can copy that info onto the Supplement.

Good luck getting the papers out and hopefully you'll have a speedy approval.

Sept. 9/08... SSA office to change status to "U.S. citizen"

Oct. 8/08... Applied in person for U.S. Passport

Oct. 22/08... U.S. Passport received

DONE!!! DONE!!! DONE!!! DONE!!!

KAELY (K-2 Visa)

Apr. 6/05... DS-230, Part I faxed to Vancouver Consulate

May 26/05... K-2 Interview - APPROVED!!!!

Sept. 5/05... Entered U.S.

Sept. 7/05... I-485 & I-131 sent to CLB

Feb. 22/06... AOS Interview - APPROVED!!!!! (Passport NOT stamped)

Dec. 4/07... I-751 sent to NSC

May 23/08... 10-Yr. PR Card arrives in the mail

Mar. 22/11.... N-400 sent to AZ

June 27/11..... Interview - PASSED!!!

July 12/11..... Oath Ceremony
