"Ensure it is notarized (by a US Notary Public), with all required supporting evidence."

Is this referring to the I-134 itself or the evidence to be attached with it? I received a letter from my bank, Washington Mutual, and that was notarized by them.

This used to be for the I-134 itself but it is no longer required.

Bank statements are not even required if you meet the minimum requirements.

Ok. I am going to provide that bank letter and a letter from my job. I am over the requirement, so I hope they do not give me any other issues about getting anything notarized.

No, they should not. In fact, there is not even room for it on the updated form. This, however, is a recent change so not everyone is up to date with it. ;)

How about paycheck stubs and tax transcripts? Do you have those as well? You should check with others from DR in regards to what is required by you. Perhaps even contact the Embassy yourself and ask.

They will most likely require at least one year's tax transcripts. Because we are into the new year they MAY require 2008. Best to contact them and ask.
