Timeline

We are in the process of completing our applications for AOS and we have come across some issues related to the I-693 or DS-3025. We do not have a copy of the DS-3025 because all the documents were handed over to POE officer.

Our questions are:

1) Is it possible to get a copy of the DS-3025 and how?

2) It seems in other posts that you are able to contact a Civil Surgeon office and they transfer your DS-3025 information to I-693A. How does the Civil Surgeon get the information from the DS-3025?

Any answers to this medical examination steps will be a great help.

What evidence do you have of required vaccinations? If you have proper evidence, you can get that transcribed directly to an I-693a, and no DS-3025 should be needed.

Apr 30, 2008 -------> Forms received by USCIS

May 06, 2008 -------> Cashed check posted to account

May 10, 2008 -------> NOA1 received for EAD, AP, and AoS

May 10, 2008 -------> Biometrics appt date received

May 28, 2008 -------> Biometrics for EAD & AoS

Jun 11, 2008 -------> AoS case transferred to CSC

Jul 05, 2008 -------> AP Approval

Jul 09, 2008 -------> EAD approval

Jul 14, 2008 -------> EAD and AP received

Jul 17, 2008 -------> AoS approved (card production ordered)

The only evidence we have is her original vaccination copies that she received in Brazil. All of them has been translated to English.

Were we supposed to receive a DS-3025 copy from the medical exam doctor in Brazil?

We are located in PA and I am not sure what Civil Physical could provide answers ot our questions.

Could the original vaccinations be approved evidence for filling out the I-693A?

I seem to have asked this question in 3 previous posts and tend to get skipped over, so I hope this individual has more luck than me. The USCIS

website is of little help in this situation. Other than the translating part, I'm in the same boat as you kwreng; the POE officer took all her documents

as well. I didn't realize we were supposed to have some back after the check-through.

Most people (us included) have used Dr. L.E. Arnold to transcribe the I-693a. In my correspondence with her, she said that we just needed evidence of vaccinations, so I'm guessing she will accept the vaccination copies as evidence. I suggest contacting her to ask if your evidence meets her requirements.
