Hello - I filed an EAD with my I485 and I130. I am renewing my EAD and need clarification on answering question 11 - any help would be great!

Question 11 asks "Have you ever before applied for employment authorization from USCIS?" I answer YES.

QUESTION #1 - What do I input for "Which USCIS Office"? I've read differing things on the internet - my forms were sent to the Chicago lockbox and my card number starts with MSC so I believe I enter "Missouri Service Center".

QUESTION #2 - What do I input for the "Date"? I am not sure what date - the receipt date? The valid date? I tend to think the valid date makes the most sense.

Why you applying for a renewal?

AOS Process

May 29th 2008- AOS/EAD/I-130 Sent to Chicaogo Lock Box sent via Overnight Delivery

June 6th - Both checks cashed!!

June 25th - Biometrics appointment

Sept. 19th - Card Production Ordered! (113 days since filing)

Sept. 25th - EAD Received!!!

Dec. 5th 2008 - Received appointment letter

Feb. 4th 2009 - AOS Interview (Interviewer says going to recommend for approval)

May 15th- called and talked to an officer to submit a service request

June 10th- Made an infopass appointment,

June 15th- Info pass appointment, told me they could do nothing until the officer made their decision!

June 27 - Filed for I-131 as well as renewal of my work authorization.

July 23rd- Received I-131

Dec 22, 2009 - Biometrics appointment yet again.

January 21, 2010 Second interview

January 22nd - I-485 Touched

January 27th - Received a Request for additonal evidence.

May 27 I-485 Approved

June 13th 2010 - Green card received!

01-17-2014 Check cleared

01-24-2014 Received NOA dated 01/14 in the mail

02-20-2014 1st Service Request created for delayed biometric appointment letter

03-01-2014 Received 'yellow' letter in the mail

03-28-2014 2nd Service Request created since the 1st request had no response from the assigned Service Center

04-07-2014 Received biometric appointment letter in the mail

04-25-2014 At last, my biometric appointment!!!!

06-09-2014 Interview - recommended for approval!

07-17-2014 Oath Ceremoney USCIS-Orlando Office! I'm A United States Citizen

02-23-2009 Date of NOA1 (AOS & EAD)
