Do I need any information about my fiance for my medical interview???

Hey everyone! I am having my medical exam next week. I have heard that I will need to fill out some information about my U.S. Citizen fiance. Can anyone tell me what information I will need about him to complete the form? His date of birth? His SSN#? His address? His phone number? Will I need this information? Any other information I need?

Eto na lang mas presentable. Hehehe.

Claudeth didn't need anything at the medical but they asked her questions about my family at the interview.

You will need his address & home/mobile number in filling out a form in SLEC. Know your possible POE too because it will also be asked. Good luck :thumbs:
