Im planning to file for AOS asap, but requirements are confusing like

1. passport(biographical)- so the photocopy of passport is ok but im planning to get a new passport which should have my new married name , is this right ? or will just stick to my old maiden name passport and pass it together with entry record stamps?

2. about the medical record, it says there that as k1, i will only submit the vaccination record, what's that? i thought i already submitted my medical record during my POE together with my visa packet, what i only got here is my vaccination documentation worksheet given by my physician after i completed my medical exam at st lukes.

3. birth certificate, is it the NSO copy?

as of now i cant think of any questions but sure will have more

merry christmas everyone and thanks very much guys

Im planning to file for AOS asap, but requirements are confusing like

1. passport(biographical)- so the photocopy of passport is ok but im planning to get a new passport which should have my new married name , is this right ? or will just stick to my old maiden name passport and pass it together with entry record stamps?--PHOTOCOPY OF YOUR PASSPORT, IN MY CASE I SUBMITTED COPIES OF THE FRONT PAGE, PAGE WITH FIANCEE VISA AND PAGE WITH STAMPS. BETTER IF JUST MAKE A COPY TOO OF ARRIVAL/DEPARTURE CARD.

2. about the medical record, it says there that as k1, i will only submit the vaccination record, what's that? i thought i already submitted my medical record during my POE together with my visa packet, what i only got here is my vaccination documentation worksheet given by my physician after i completed my medical exam at st lukes.--YES , THATS IT! THAT VACCINATION DOCUMENTATION WORKSHEET. SOME PEOPLE JUST SUBMITTED THAT WITHOUT THE I-693. IN MY CASE I SUBMITTED THE VACCINATION DOCUMENTATION WORKSHEET AND i-693 WHICH IS TO BE COMPLETED BY A CIVIL SURGEON.

3. birth certificate, is it the NSO copy?--YES AND IF THE NSO COPY IS NOT THAT CLEAR , MAKE A COPY OF YOUR ORIGINAL BIRTH CERTIFICATE TOO

as of now i cant think of any questions but sure will have more

merry christmas everyone and thanks very much guys
