Question on how to fill out one answer on G form for K1

I am wanting to make sure I do this right and I have a question about the G325-A form concerning the last five years of employment for me..

I am self employed- I work as an independant contractor so I do not receive w-2s of any kind. I do contract work and from some of the contracts over the past five years I have gotten W-9's. On this form- do I simply put that I am self employed and have been for the past five years or should I list all of the companies that I have received W-9's from over the past five years?

I am self employed, receive 1099's.... I simply stated that I was self employed, the kind of work I do (anything and everything computer related) then listed the company's I do the most regular contract work for.

For the I-134 stage, I also included a letter from this Company stating I had guaranteed work. (this was suggested by NZ the Embassy)

Self Employed will be fine.

No need to include everyone you have rec'd a W-9 from

[payxibka 10,044]

I hope you mean 1099's as a W-9 is Request for Taxpayer Identification Number and Certification and does not indicate employment.
