Hi, if i remember correctly , its the DS 156, DS 156k and DS 157, plus the appointment letter.

They use to mail you a packet with outdated forms but now they dont. We didnt use anything from the packet except the appointment letter. When you get your appointment letter it will list what you need to bring.

You have to pay dv at metrobank and pay your visa at bpi. Bpi have there ds-157 forms wherein they print at the back that they recieve your payment (photocopy it). Print out The ds -156(2 copies) and ds-156k(1 copy) from usem website. Your fiance must send you ITR, W2, I-134,and phone bills
