Hi!

After the wedding i sent that form (ss-5 i think) to get another social security card with my new name on it, but i got it back about a week later saying that they didn't receive enough evidence of the name change. I can provide them all the necessary documents but one: they want another document that has my new name on it and i don't have one. My SSN was going to be the first one...

I'm confused

What did you send for the new SSN card?

You must send your marriage license showing your new married last name, they will send it back to you.

so it's just the SS-5 form and the marriage licence?

fill out another SS application and go to the local office with marriage certificate. Here in Michigan marriage certificate does not state the new name so we had to fill out the form with the new name and explain the situation. No problems and entered it as on the form we filled out. I think just mailing it in is not enough.
