HI all. Does anyone know, on the form DS-156, question #4, what do they mean when they ask "How and when were you divorced"? I understand the when of course, but what do they mean by how?

Actually, the previous question was for form DS 156K.

Also, on DS-156, question # 18, it asks the spouses name. If he is divorced, do I have to put her name here?

The first question, i.e., "how divorced?" -- was it decreed by a state or local court (which one) or was it a quickie obtained somewhere else.

The second question, yes, the ex-spouse's name should be included. Helps USCIS in the event of some reason to verify the divorce.
