I-134 Important Question Please Advise

I just got my I-134 Notorazed today and have an important question that hopefully somebody can help with. The Notary attached a California Jurat with Affiant Statement with the pertinent information and told me that the I-134 notary section (back side) is outdated and they cannot sign on the I-134 form. So this attached form has the subscribed date, stamp, etc and its stapled to the I-134.

The notary told me that the I-134 she is no longer allowed to sign on that form because it is missing one line and they have been informed by immigration to attached the form I described above. Can anybody confirm a similar situation like this? I am confused but she assured me this is the policy and for her to sign on the I-134 outdated form would be illegal. The new form wont be ready for another year so this is why they are doing this. Please advise, thanks. I will likely go to another notary and seek a second opinion on their policy.
