please help me..i just want to clarify...in forms G325A and I765 theres a portion asking "Other Names Used (Include Maiden Name)" what will i put? my maiden name only like the example here in VJ or i will put my whole name like what is written in my passport?

example:

Other Names Used (Include Maiden Name)

dela cruz

or

Other Names Used (Include Maiden Name)

maria catindig dela cruz

please enlighten me

if you've already put your legal name on the forms ... all u need to do is put 'dela cruz' in other names used

Any legal name you've ever used (ie: has been on any passport, birth certificate, drivers license, etc)

For me, since I had been previously married and taken my first husbands name, we had to list both my maiden name and my first married name. Hence:

Current name: Sally Giselle Smith

Previous names: Sally Giselle Taylor (previous married name); Sally Giselle Wallace (maiden name)

but its different in my country ...our maiden name will be our middle name once we get married....for example

maiden name: maria(first name) catindig(middle name) dela cruz(last name)

married name: maria dela cruz bush

so i will write "maria catindig dela cruz" in "Other Names Used (Include Maiden Name)"

am i right?

If that's what has been on your previous legal ID, then yes - that's what I'd put!

hi i put mine like this:

Primie Galenzoga Romano --- other name used (single name) ---- this is my name in my passport..

Primie Romano COmbs --- married name (i used in all forms in aos, ead, ap, ssn, state id, driver learners permit, insurance, bank.. etc)

--- i did not dropped my father's family name coz i don't want conflicts if I need to get new passport with married name from Phil..
