Question about this form

I received this form along with one half of my original interview letter and a pink slip. Just curious about this form. Has anyone else received the same thing? Thanks...

hey maricor! that's what im telling you! that's MNL IV 22 or 221g aka white slip..

that means USEM will still have to wait for the document verification.

that's the same form i got.

ah ok so eto pala ung white slip..eh anu pa ung nakita ko sa iba na white paper na document verification ang nakalagay wla naman nabgay sakin na ganun..wla din ba chinek sayo ganyan din tlaga itsura? hehehe thanks ha parehas pala tayo... yan lang kase at ung half ng apointment leter ko kasma ng pink slip...

Nasa left bottom ng papel ung nakasulat MNL-IV-22. ganito ung binigay sakin. ang my check is FROM A QUALIFIED CO SPONSOR. Under nun, ung 1st two requirements ang hinihingi, I864 at income tax. Taz sa baba under ng what to do next, ang my check is WHEN YOU HAVE ALL...

Hay buti pa kayo sitting pretty na lang. Congrats nga pala! American citizen to be na ang baby mo.
