The medical fee and other fees are listed here: http://manila.usembassy.gov/wwwh3023.html

I think there are links on the VJ guides also.

Good luck on your medical and interview.

Hello guys,

I had my medical May 2 (Friday) and May 5 (Monday). First day was okay. Went at St. Lukes after lunch and finished before 5pm. On the second day, I had 4 shots of vaccine.(TDAP, MMR-Measles-Mumps-Rubella, Human Papillalomavirus and Varicella). 2 for both sides of my arms. My right arm was okay now, but my left arm is very sore. I have fever since last night until now. So am not really feeling well tonight. But dropped here coz i wanted to share this to you guys.

I brought the following:

1. appointment letter (printed from my email) - where there is only "confirmation number" of the scheduled interview on it.

2. printed IV schedule from the site IV Schedule-where it has your "case number", the guard asked me of this, so i suggest it's better to bring yours also.

3. 3 pcs. 2x2 pictures (1 pc for first day , and 2 pcs. for second day)

4. Medical Fee (as of May 1, 2008 new fee is P9,704.05)

I have asked few times and not less than 5 times about when was the first day of my last menstrual period, so it's better to don't forget when you had your period last month. I was asked also few times in every exam i went in about complete name, birthday, age, (this is to double check also what they write and what i wrote in my forms, to avoid errors of the name, birth date, age. ) Basically they asked about the infos written on our passports except of course the passport number)

Basically, the food is not allowed. If you will notice, there are cameras in every floor. Even if you had brought food inside without the guard noticing it, but they have security monitoring the camera, so once the security saw at cam that someone is eating inside, the guard will come to you and warned you that food is not allowed. Well, the point is we don't want to be embarrassed in front of many people right?

Steps i have done on the first day:

1. showed the appointment letter and IV schedule to the guard.

2. The guard gave me forms to be filled up. this form i pasted my 1 pc. 2x2 pic.

3. After filling up , i gave the forms to the front desk/ reception counter at ground floor.

4. Then, the front desk instructed me to go inside the digital room and had took a pic of me for their database record.

5. After they encode all my details in their database, they gave me the forms i already filled up and instructed me to proceed to the cashier 5th floor and pay the medical fee. (payment should be made in Philippine peso and not dollar)

6.Urine/Blood test (4th floor)

7.Xray (3rd floor)

8.Vital signs, height and weight determinations (5th floor)

9.Eye test (5th floor)

10.Medical exam where you will go to inside the doctor's room and all applicants are asked to disrobe completely). Don't worry it would be only you and the doctor inside the room, beside if you are a girl , a female doctor will check on you and male doctor for a guy.

(here , you will only be asked based on the forms you already filled up. The questions were already in forms we filled up for Physical Exam.)

After she asked questions, she then checked my naked body and list the marks she saw, like the birth marks. And if she saw something like scar, she will ask you where this scar came from.

I was inside for less than 5 minutes. This depends on the person the doctor is checking up, i noticed senior citizens or those old women been in room for 15 minutes to 20 minutes.

11. After the doctor checked me, she instructed me to go to Immunization Interview (2nd floor)

12. At immunization interview, the doctor asked me if i had chicken pox before and i said yes, and she asked me to show my scar, i showed my scar th my eyes brow which i forgot i had thick eye liner, so she laughed asked me "where"?, she asked me if i have mirror and show her where, i laughed too and i said ops, sorry Doc, i forgot i have eyeliner. hehe. She then asked told me okay just tell to the nurse on Monday who will give you vaccine where the chicken pox scar is. I said Ok doc.

She asked also where i want to have my vaccination here or in US. i answered here would be fine with me. And she said ok. then I was finished at Immunization interview. She then instructed me to go to Check out counter.

13. I gave the filled up forms to the check out counter lady and she gave me back my passport and receipt, and stub for the result.

14. Done

SECOND DAY:

1. Had vaccines at Immunization Room.

2. Result at ground floor.

3.Finished.
